Apply for Equine Facilitated Therapy

1. Please read through the application documents, and our FAQs
2. You can self-refer, by downloading our Participant Application Form, Cover Letter and Doctor’s Declaration, filling in all sections and sending it to us.
3. Alternatively, you can be referred through your GP, Physiotherapist or other Healthcare Provider.
4. Once we receive your form, we will be in touch to invite you to our Centre, for an assessment with our team, where we will create a therapy plan for you to achieve your individual goals. But you might have to wait a while to start, as we have lots of people who want to come!
